Fifth Grade "Graduates" Health Requirements
Cherry Hill School District Policy and NJ State Law mandate that all eleven year old and older students entering sixth grade must receive meningitis (Menactra) and pertussis containing (Adacel or Tdap) vaccines. Students who are eleven years old and do not present documentation of having had these vaccines will be excluded from school on the first day in September and will be until such documentation is received. If a child's check-up is scheduled for a date after the first of September, or their birthday occurs late in the calendar year, you can submit a copy of the appointment card or a document from the healthcare provider indicating the date and time the student has an appointment.
If a student is anticipating participating in athletics in middle school he/she will be required to have an athletic physical exam recorded on the approved state form. You can submit both the immunization record (or the document of a pending appointment) and the athletic physical exam report to the middle school nurse after June 1 of the fifth grade year, or submit to me before June 1.
